To obtain a copy of your medical records, please follow the instructions below.  

1. Print and fill out this form: MS Authorization for Release of Medical Records 

2. Send the completed form with a copy of your government ID to medical.records@mercyships.org or bring it to one of the locations listed below:
a) A Mercy Ships National Office
b) One of the ships – Global Mercy or Africa Mercy

3. Once we receive your request, you will get notification from medical.records@mercyships.org verifying receipt.

4. Please allow 30 days to complete the request and have the records sent.
